 I have provided the following forms for your convenience. Take your time filling out your initial health intake form before your first appointment and save a little time. You will only need to fill this out for your first appointment. I have also included my cancellation policy. Please read carefully and sign. A copy will be made for you and the original kept in your records. This is the form to use if you are coming in for a prenatal massage instead of the TBMT Intake Form. If you have health insurance please download the PDf files here for your first visit. These are necessary in order for us to bill your insurance. If you have not already done so, please call the office and have us check your policy to be sure your benefits cover massage therapy. We will need a copy of your drivers license and the front and back of your health insurance card. Please bring these completed forms with you to your first appointment. The HICFA 1500 form only needs your signature in the middle of the document. We will fill out the rest. You can wait to sign this document at your appointment.
